Capitvate templates for an annual attestation/certification

I’m looking to develop an attestation form with captivate where people have to read a page then click a button to “confirm” they have read it and will comply etc. does anyone have any templates I can see to get some ideas. I know its a simple project, but I’m a newbie

Lieve is correct. Adobe Captivate is primarily a tool to create training for an LMS. Of course, it doesn’t mean that it can’t be used for other purposes. A meter stick (Yard Stick) is used for measuring distance but teachers sometimes use it to wack the desk to get everyone’s attention or sometimes as a pointer to call on particular students.

When I worked for my last employer I used Adobe Captivate to capture an annual policy sign-off for a code of conduct. This saved my company from gathering signed forms and storing them in filing cabinets unnecessarily.  By publishing to the LMS a three slide course that accepted the user agrees that they read the policy and were bound by its terms, we saved all that paper but also were able to generate completion reports as to who had or had not completed this yearly activity. We did have to run this past the legal department but they were in agreement that so long as there was a button that was labelled “I AGREE” that completed the course they were fine with it. Here’s how I did it…

The first slide contained the document in a web object (PDF file). At the bottom of the page was the I AGREE button that simply took them to the last slide. On that slide was merely a thank you message and instructions to close the course.

Before you publish make sure you set up reporting as I did…

  1. Click the Quiz dropdown menu
  2. Click on Quiz Preferences
  3. Enable reporting for this project
  4. Select the Standard (in my case it was SCORM 1.2)
  5. Set Status Representation as Incomplete –> Complete
  6. Set Success/Completion Criteria to Slide Views and/or quiz
  7. Select Slide views (not Quiz) and set it to 100%
  8. Click OK and publish the project as you would normally.

This way when an end-user completes their annual sign off they are considered complete in your LMS when they have clicked on the “I AGREE” button that merely takes them to the second slide and therefore they have viewed all the slides in the course. I am hesitating with an answer. Captivate is an eLearning authoring tool, what you want to create doesn’t have anything to do with Learning or do I miss something in your question? For such a purpose I would use a pdf form, with a digital signature. Why do you want to use Captivate?  It is easy to create a slide with text and add a button to it to be clicked, even to attach a score so that it can be transferred to a Learning Management system.  A template file in Captivate is a cptl file, and has no sense in this case.

Maybe you could clarify more? Also tell which exact version of Captivate you are using? If it is 11.5, have a look at the included Quick Start Projects under the Assets panel. You’ll see lot of slide types, maybe you can use one of them?
