Trouble with defining the correct/incorrect answers and actions for answer for quiz

Can you please offer some more details? Which exact version do you use, full number is mentioned under Help, About Captivate.
Each question slide is based on a quizzing master slide. Master slides are part of a theme. Which theme do you use in the project? Did you edit the quiz slide or the master slide? Deleting embedded objects (without an individual timeline) can cause serious problems. Did you delete some of those items? Have a look at more explanation in:

Quiz slides, if the attempts are finite, has two possible actions: Success (for correct answer) and Last Attempt. The second one will only be done when all the attempts are exhausted and always have been Failed. Their default value is set to Continue. Which actions did you choose?
